Clinical practice and research of acupuncture analgesia

Abstract

針灸已在中國使用幾千年,目前在世界各國也廣泛使用,針灸可以用來治療多種疾病,但以治療疼痛最為人熟知。過去幾十年來的研究,我們已經逐步瞭解針灸的機轉,在週邊的針刺或電針刺激可傳導神經衝動至脊髓或經由上升徑傳送至腦部而刺激中樞神經系統釋放神經傳遞物質如:腦啡肽β-內啡肽或單胺類神經傳遞物質如:血清素或正腎上腺素,因而產生止痛作用。我們研究發現:以交替模式進行電針電刺激比同步模式進行電刺激較不會產生電針耐受性針灸,因此建議以交替模式為病人治療。

除了基礎研究之外,許多研究者也著眼於針灸臨床試驗,近來隨著研究方法的進步,已經使臨床試驗的結果較為可靠,我們的研究發現針灸或電針可以有效緩解手術後疼痛並,減少嗎啡引起的副作用,並有效緩解五十肩痛、關節痛、下背痛、剖腹產引起、震波碎石之疼痛等。

Acupuncture has been used for thousands of year in China and widely accepted by the neighboring countries. It is applied to several kinds of disease; however, pain relieving is the most remarkable indication worldwide. In the past decades, the mechanism of acupuncture analgesia is the central topic of acupuncture study. Recent studies and our results indicated that electroacupuncture facilitates the release of certain neuropeptides (i.e. enkephalin and β-endorphin) and monoamines (i.e. serotonin and norepinephrine), which in turn produce anti-nociceptive effects. Electroacupuncture stimulation with alternating mode yielded better results than with simultaneous mode for treating chronic pain because it may postpone the development of EA tolerance. We suggest use alternating mode to treat patients.

Asides from the basic studies of acupuncture, many researchers focus on the clinical trial. Recently, the development of methodology makes studies more reliable. Our results suggested that the efficacy of acupuncture/electroacupuncture on pain relieving and on reducing side effects of morphine administration after surgery. Besides, acupuncture/electroacupuncture also showed efficacy on pains due to frozen shoulder, knee-joint, lower back, cesarean section, extracorporeal wave shock  lithotripsy etc.

       (1)   In 2014, Dr. Lin was appointed as an expert and consultant by UNESCO, and was once again invited to participate in "The 9th Session of the Intergovernmental Committee for the Safeguarding of Intangible             Cultural Heritage” held in Paris, France, where he delivered a lecture in the ICHNGO Forum.

      (2)    In 2014-15, Dr. Lin obtained the highest ranking of research performance in Taiwan in the fields of Complementary therapy, Acupuncture points, Meridians, and Electroacupuncture; top 9 rating in the  world, in acupuncture point research field, as evaluated by expertscape.com.

      (3)   In 2015, Dr. Lin was invited to attend the sixty-sixth session of the World Health Organization Regional           Committee for the Western Pacific held in Guam, United States of America.

     (4)   In 2017, Dr. Lin was invited to attend the 140th session of the WHO Executive Board in Geneva,                          Switzerland, where he delivered a lecture entitled “The Use of Acupuncture in Emergency Medical                      Services”.
